A 79-year-old man with a history of hypertension for which he is receiving a beta-blocker and an angiotensin-converting enzyme (ACE) inhibitor presents to his new primary care provider for a routine physical examination. He is unable to provide a reliable history, although he remembers that 5-6 years ago he was admitted to the hospital and underwent a procedure for a heart-related problem. His pulse is noted to be irregular and a 12-lead ECG is obtained.
